Important Update on Sickness and Accident Benefits

The City has made an important change PDF File in how employees apply for Sickness and Accident (S&A) benefits during the influenza (flu) season.

Effective October 26, 2009 through March 28, 2010, employees who are eligible for S&A benefits through MEBAC, who have fluĀ©\like symptoms and are absent for greater than 5 working days, can fill out and submit an Influenza/H1N1-Swine Flu Employee Self Assessment  PDF File and ECM Referral Form External Site. instead of an Attending Physicians Statement (APS).
