This is a witness statement form and does not substitute for calling the police to report a crime. A police report and case number must be in place prior to filling out the witness statement form.

The Calgary Police Service investigator assigned to your case will direct you on where to send the completed witness statement form.

Please ensure your witness statement is specific to the offence you have reported and/or witnessed. The purpose of your witness statement is to capture evidence and information pertaining to a single event.

The law requires that the witness statement provided be disclosed to the accused and/or defence counsel, which is the responsibility of the Crown Prosecutor’s Office when a charge is laid. Witnesses are under no obligation to speak to defence counsel or the accused.
