How to apply

There are multiple ways you can submit your Fair Entry application. The instructions for each are found below.

You can apply using our online application form.

Before starting the online application, make sure you have your required documents.

You can apply in-person at one of the following locations below.

You can apply by mail by sending your completed application and required documents to:

You can apply by fax by sending your completed application and required documents to:

You can drop off your application and required documents at any of these locations.

Place your completed application and required documents in an envelope with “Fair Entry” written on the front (no postage necessary) and it will be sent to Fair Entry for review.
