The City is committed to protecting your personal information and implementing reasonable security arrangements to prevent unauthorized access, collection, use, disclosure or destruction. Despite these safeguards, there may be circumstances where personal information is compromised. When this occurs, this may result in a privacy incident. 

What is a privacy incident?

A privacy incident occurs when there is a loss of, or unauthorized access to, or use of personal information.

If you believe The City has improperly collected, used, or disclosed your personal information, you can submit a Privacy Incident Report. 

Please be specific and provide as much detail as possible about the incident, including the specific business units or departments you believe were involved.

Note: Do not include personal information about third parties in your report. Your submission will be assigned to a member of the Access and Privacy team, who may contact you for additional information or evidence, if needed.
